SERVICES

One Sky Community Services assists with access to systems of care provided by New Hampshire’s Department of Health and Human Services (DHHS). Working with individuals and their families or guardians to apply for services through the Area Agency System and/or New Hampshire Medicaid.

Eligible services vary based on age and individual need. ​ Eligibility criteria and intake processes are different in each of three categories:

1.     Family-Centered Early Supports and Services (ESS)

2.     Services for Persons with Developmental Disabilities

3.     Services for Persons with Acquired Brain Disorders

Once a person has been determined eligible for Medicaid Services One Sky provides Service Coordination and Administrative services. As an Area Agency One Sky provides access to Residential Support, Respite, Service Coordination, Specialty Funding, Community Support Services, Vocational support, In-home support and many other services.
