Targeted Case Management - Transition Services (TCM)

It can be difficult to navigate the transition of children centric services and waivers systems to adult based services. Young Adults 18 to 21 years of age can be served by multiple funding sources. Understanding the limitations and purpose of these funding sources can be overwhelming.  One Sky Community Services provides Targeted Case Management services designed to help Young Adults transition to adult services.

Family Support Services (FSS)

FSS services are designed to provide families with the support necessary to keep their families intact and prevent the need for more intrusive, restrictive and costly care. We provide transitional support services to children, adolescents and young adults from ages 3 to 21 years old. These individuals are diagnosed with a developmental disability and are living with a family member. As the person matures through their life and into adulthood, we provide guidance and education related to the services and community resources that are available to them and their family members. This includes:

  • Assistance with applying for state and federal benefits

  • Working with you to plan for transitions from early supports and services to school and then adult services

  • Individual and Family Advocacy

  • Accessing home modifications or adaptive equipment

  • System(s) information and referrals to providers

  • Recreational opportunities

  • Parent to parent support

  • Respite services

In-Home Support Services (IHS)

IHS is offered to individuals from birth to 21 years old that have been diagnosed with a developmental disability, face significant health or behavioral challenges as defined by statute, and are living with a family member. The IHS program is designed to provide support to the individuals natural care givers that are necessary to maintain that person within their home and community while developing critical skills that increase their independence and community involvement. Funding that is provided is directed and managed by the individual or representative. Services included:

 

  • Personal Care Services

  • Service Coordination

  • Transitional Supports

  • Environmental modifications

  • Assistive technology

  • Therapeutic recreation Services

  • Assistance with applying for state and federal benefits;

  • Assessment and support

  • Individual and Family Advocacy;

  • System(s) information and referrals to providers;

  • Recreational opportunities;

  • Parent to parent support;

  • Respite services
