SERVICE COORDINATOR – Full Time- Portsmouth, NH 

Do you have a Bachelor’s degree in a Human Services/related field, and do you want to make a difference in someone’s life?

If so, One Sky Community Services is looking for a Service Coordinator to enrich the lives of our clients!

This is a full-time position which includes eligibility for our organization’s medical and dental plans, 11 paid holidays, and a generous amount of paid time off that begins accruing your first day.

Responsibilities of the Service Coordinator:

  • The Service Coordinator is responsible for evaluating and determining clients’ needs for services.
  • Service Coordinator ensures overall compliance, quality, and cost budgeting of service delivery.
  • Service Coordinator is responsible for prioritizing and negotiating needed services in ways that are appropriate, cost effective and quality driven.
  • Work schedule will vary in order to accommodate the individuals served, their families, program schedules and department/agency events. These duties will require the service coordinator to spend considerable time on the phone and working away from the office setting.

Requirements for this position:

  • Bachelor’s degree or equivalent in Human Services or related field
  • Minimum of 2 years work experience in related position with responsibility for developing community, family and client relationships, creating service plans, negotiating and budgeting resources, quality assurance, compliance, or other comparable areas.
  • Previous experience in budgeting, cost accounting, or other areas of finance.
  • Must be able to physically access family homes, community buildings, program sites, etc.
  • Must have own transportation. A criminal record, driver record and proof of auto insurance coverage of 100/300 is required; employment is contingent on satisfactory results.

INTENSIVE TREATMENT SERVICES (ITS) SERVICE COORDINATOR – Full Time- Portsmouth, NH

One Sky Community Services is looking for a new Service Coordinator/ Case Manager to join our ITS (Intensive Treatment Services) program. This position is a full-time 40 hours per week position which is eligible for all employee benefits. We offer health insurance, dental coverage, 11 paid holidays, 4 weeks of paid time off your first year of employment, and mileage reimbursement to our employees.

The Service Coordinator position is one of the most important positions at One Sky. As a Service Coordinator you are the key in helping the individuals we serve achieve their dreams.

This position focuses on 4 key areas of the “whole person”:

  • Ensuring that the individuals we serve have a safe and secure place in which to live;
  • Helping them integrate into their community where they can develop healthy,productive relationships with friends and peers;
  • Assisting them in finding and obtaining employment or other productive interests;
  • Identifying and implementing supportive solutions to obstacles that prevent them from living meaningful and productive lives in our community, e.g. transportation

The Service Coordinator is responsible for:

  • Identifying what those dreams are
  • Assessing each individual’s capabilities to achieve those dreams on their own
  • Identifying the gaps in each individual’s capabilities/resources
  • Developing and leading the effort to implement a plan that addresses those gaps; which includes:
  • Prioritizing and negotiating needed services in ways that are appropriate, cost effective and quality driven;
  • Ensuring that all services are delivered in compliance with agreed upon service agreements, budgets and consistent with all applicable rules and regulations

As an ITS Service Coordinator you will be responsible for assisting and supporting a caseload between 15 and 20 of our individuals with the greatest needs and greatest risk of harming themselves or others in the community. You will be working with our service provider partners (vendors) to ensure that the needs of those individuals are being met and that we are doing so consistent with our internal policies and procedures and all applicable state and federal guidelines

Requirements for the ITS Service Coordinator:

  • A Bachelor’s Degree in a Human Services related field (ie. Psychology, Sociology, Social Work, etc)
  • Minimum 2 years’ experience performing case management or service coordination.
  • Experience working with clients with high-risk behaviors, mental health conditions, or substance abuse treatment.
  • Must demonstrate a high degree of compassion and understanding with a strong desire to help people
  • Demonstrated ability to establish and maintain trusting relationships with the individuals we serve, their families and other stakeholders. Serve as a trusted advisor and consultant to the individuals you are assigned to serve. Guide the individuals you serve and their guardians through the process of obtaining the supports and services they will need to be successful
  • Proficiency in office software applications; primarily the Microsoft Office suite of applications but other cloud enabled applications as well.
  • Strong priority management , planning and organizational abilities

FAMILY HIRED DIRECT SUPPORT PROFESSIONALS – PT/FT – Northern Rockingham County

Our mission at One Sky Community Services is to assist people with developmental disabilities and brain injuries to live as valued and participating members in their communities.

With your help, our clients become stronger, more independent members of society.

We currently have several openings for family hired direct support professionals to assist and support our clients and their families throughout northern Rockingham county.

We serve children, teenagers and adults with developmental disabilities and/or acquired brain injuries.

These positions are typically part time and would be ideal for as a second job, current college student. etc. since most families are flexible about the work schedules.

Most positions involve taking clients out into the community to attend/participate in pre-planned activities such as (but not limited to): swimming, bowling, shopping, music events, sporting events and anything else that is of interest of the particular client and in line with their goals.

The ideal candidate would be creative, patient, outgoing, caring and have excellent judgment. Experience working with people with disabilities is a plus!

Some clients require personal care and wheelchair transfers.

The pay rate for these positions vary, but typically pay a higher wage than most DSP positions elsewhere.

Candidate must submit to and successfully pass a NH Criminal Background Check, driving record check and BEAS (Bureau of Elderly and Adult Services) check.

All staff are also required to have reliable transportation and automobile insurance. These positions typically require the use of the employee’s vehicle to transport the client to various community activities.

A state mandated Tb test is also required at no cost to the candidate after an offer has been made.

DIRECT SUPPORT PROFESSIONAL – PART TIME – Portsmouth, NH

A Portsmouth, NH family is seeking someone to assist in the care of their 12 year old son with Autism.

Duties would include assisting in activities of daily life such as: bathing; toileting; tooth brushing; cleaning his room; cooking/baking; practicing social skills in the community; and from time to time going to the pool to practice swimming – with floatation devices.

The benefits of this position are the joy of helping a loving, kind, young boy with Autism learn everyday skills.

Experience working with individuals with Autism a plus, but we will train the right candidate.

Hourly wage is $12/hour; 10 hours per week

MENTOR AND ROLE MODEL – PART TIME – Rye, NH

A family located in Rye, NH is seeking a positive role model and mentor for their male pre-teen son with a developmental disability.

The ideal role model/mentor candidate would be able to provide a positive but firm approach and be willing to use multiple “tools” to motivate the client. The role model/mentor will provide transportation and support to client during client’s community activities. The role model/mentor would also be able to provide general assistance to client with frequent interactions with his siblings, with the goal of a positive outcome.

Flexible schedule, but primarily between the hours of 3:30p-7p during weekdays, with occasional assistance on weekends would be ideal. Depending on the availability of the candidate, days worked would range between 2 or 3 days per week. This is a part time position; approximately 9 hours per week.

The successful candidate for this Role Model/Mentor position would possess experience working with people with disabilities, be patient, positive and firm, be able to model positive behavior both in public and while interacting with family.

Candidates must have reliable transportation, a valid drivers license with a good driving record, a current auto insurance coverage and be able to successfully complete a NH Criminal Background Check, BEAS Check and Driving Record Check. We are looking for an enthusiastic, energetic, creative, and compassionate person to join a young woman from Nottingham NH who is affected by a developmental disability. You will be providing one-on-one home and community-based supports. You must be willing to explore opportunities wherever they may lead, enjoy meeting new people, and believe people with disabilities can and should lead full happy lives.

This position requires a high school degree or equivalent. The ability to speak, read, and write English proficiently as documentation is required. All applicants must have a valid NH driver’s license, auto insurance and submit to a criminal record, Bureau of Elderly and Adult Services and motor vehicle check.

The ideal candidate must be flexible and able to work with minimal supervision. It would be beneficial to have experience providing direct support and be able to adapt to changing environments. Transportation is required for this program and will include travel throughout the surrounding areas. This is a 20 hour per week program Monday thru Friday. Scheduling has potential to be flexible but this young woman needs to be driven to work by 4pm on Mondays and Wednesdays. Rate of pay will depend on experience.

Duties include picking this young man up from school and taking him to pre-planned after school activities 15 hours per week, 3 hours per day.

A Portsmouth, NH family is seeking a support staff that can assist in the care of their 9 year old son with a developmental disability. The ideal candidate is patient, affectionate, positive and firm, and able to model positive behavior both in public and in the home.

This staff person will also need to be prepared to lift and carry ~40lbs. Their son is very small, non-verbal, friendly, and enjoys music, being held, and trips to the playground. He can walk with assistance/supervision but also uses a stroller for longer distances, such as walks to downtown. Duties would include assisting in activities of daily life such as: bathing, toileting, eating, walking, practicing social skills in the community, and from time to time going to the pool to practice swimming.

This is a part time position and the family is looking for someone who can work approximately 13 hours per week at a $15 hourly rate of pay. The schedule has opportunity to be flexible, but ideally between the hours of 5:00pm-7:00pm during weekdays, and 3 hours on weekends.

When a viable and enjoyable routine has been established the DSP will continue to support the client by encouraging interactions within the community and promoting independence.

Candidate for this position must be flexible, king, and patient (yet firm). Candidate must have a valid driver’s license and be willing to undergo a Criminal and Driving background check.

Transportation for this position will be in the client’s handicap accessible van.
