Our private non-profit organization serves 24 cities and towns in the seacoast region of New Hampshire. We’re part of a statewide system that provides and arranges supports for children, adults and their families. Our staff of 200 works to assist people in northern Rockingham County to reach their full potential, by providing service coordination and family support services. Through our Futures programs we provide trusted direct supports for high school transition, education, employment, residential and community services. Further program choices are available from 17 agencies that subcontract with One Sky.

IHS (In Home Support) COORDINATOR – FT – Portsmouth

We are currently seeking a full time In Home Support Coordinator in our Portsmouth Office.

Job Objectives: The IHS (In Home Support) waiver provides personal care to eligible children in their home. The IHS Coordinator is responsible for maintaining the wait list of children needing services, developing and overseeing budgets and service plans for eligible children, monitoring utilization and compliance with state regulations, as well as monthly service coordination and quality assurance activities.

Education Requirements:
Bachelors Degree or equivalent in Human Services or related field.

Skills Required:

  • Minimum of 2 years work experience in a related position with responsibility for developing community , family and client relationships, creating service plans, negotiating and budgeting resources, quality assurance, compliance or other comparable areas.
  • Previous experience in budgeting, cost accounting or other areas of finance helpful.
  • Ability to evaluate service needs and monitor ongoing services for cost efficiency.
  • Competency in logistics and cost estimating of services, ensuring that services are delivered in a timely, cost efficient way with appropriate outcomes.
  • Knowledge of community resources and the ability to match clients to those resources.
  • Highly effective skills in critical thinking.
  • Ability to work independently and collaboratively.
  • Excellent interpersonal skills required.
  • Strong time management and organizational abilities, flexible to meet clients’ needs.
  • Ability to work in a time sensitive, multiple deadline, constant change environment essential.

GROUP HOME DSPs NEEDED – ON CALL PER DIEM –  North Hampton & Newmarket

Work with people with physical disabilities and behavioral challenges. Seeking someone experienced with personal care for individuals with quadriplegia, using Hoyer lift, wheelchair lift and tie downs in a handicapped van, communication devices and standers.

The DSP will provide assistance to the individuals in the home and community to develop and enhance basic living skills, personal decision making, social skills, safety skills, a healthy lifestyle including good nutrition, and rights and responsibilities as citizens.

Some of the essential job functions are (additional duties will be explained during interview process):

  • Ensure health & safety of individuals

JOB COACH/MENTOR – School to Work Day Program – FT – Newmarket, NH

One Sky Futures (a service provider department of One Sky Community Services) is a provider of direct care services to adults with developmental disabilities and/or acquired brain injuries. We have a school to work day program , located in Newmarket, NH. This program is looking for reliable FULL TIME (M-F afternoons), enthusiastic, self-motivated and energetic Direct Support Professionals (DSP) to support the people we serve.

Each client is unique and has their own interests, passions and personality which requires Direct Support Professionals (DSP) that are able to respect each clients choices and support them in multiple activities and environments.

Prospective Direct Support Professionals (DSP) candidates need to be flexible and committed to supporting the clients in a way that allows them to reach their maximum potential.

As a Direct Support Professional (DSP) dependability is priority! Experience given consideration but dependability, commitment and a positive attitude more important. Must be open to providing complete personal care in some cases – training is provided.

Direct Support Professionals (DSP) will be provided with training and must pass a NH Criminal Background Check, BEAS Check and Driving Record Check. A family located in Rye, NH is seeking a positive role model and mentor for their male pre-teen son with a developmental disability. A male role model would be ideal, but all qualified and interested applicants will be considered.

The ideal role model/mentor candidate would be able to provide a positive but firm approach and be willing to use multiple “tools” to motivate the client. The role model/mentor will provide transportation and support to client during client’s community activities. The role model/mentor would also be able to provide general assistance to client with frequent interactions with his siblings, with the goal of a positive outcome.

Flexible schedule, but primarily between the hours of 3:30p-7p during weekdays, with occasional assistance on weekends would be ideal. Depending on the availability of the candidate, days worked would range between 2 or 3 days per week. This is a part time position.

The successful candidate for this Role Model/Mentor position would possess experience working with people with disabilities, be patient, positive and firm, be able to model positive behavior both in public and while interacting with family.

Candidates must have reliable transportation, a valid drivers license with a good driving record, a current auto insurance coverage and be able to successfully complete a NH Criminal Background Check, BEAS Check and Driving Record Check. All new hires must submit to an agency paid TB test, per NH State Regulations.

Hours are 8:30-3:30 Mon-Wed and Fridays. Rate of pay is $10-12/hour (plus mileage reimbursement), depending on experience.

Staff will assist him with high-school level academic work (i.e. reading, money management, etc.), finding volunteer and employment opportunities, weight management, physical activities and social activities at home and in the community. A family in Greenland, NH is looking for someone to provide a variety of daily responsibilities and activities with and for our 22 year old daughter with developmental disabilities.  Position responsibilities include some meal prep, job support, transportation, schedule coordination and engaging in peer social and/or community activities.  Activities vary per day and per week.  Our daughter is kind hearted and pretty consistently upbeat.  She loves singing to the radio, following baseball stats, swimming, cooking, crafts and enjoys staying busy.  We are looking for someone who can interact with her as the young adult she is developmentally, while supporting her navigating through her week, including her work.

Hourly wage is $12-14/hr.  Schedule is M-Thurs in the range of 7:30a-3:30p for a total of 23-26 hrs/week.

You will be providing one-on-one home and community-based supports. The ideal candidate must be flexible and able to work with minimal supervision. It would be beneficial to have experience providing direct support and be able to adapt to changing environments. Transportation is required for this program and will include travel throughout the surrounding areas. This is a 20 hour per week program Monday thru Friday. Scheduling has potential to be flexible but this young woman needs to be driven to work by 4pm on Mondays and Wednesdays. Rate of pay will depend on experience.
