Our private non-profit organization serves 24 cities and towns in the seacoast region of New Hampshire. We’re part of a statewide system that provides and arranges supports for children, adults and their families. Our staff of 200 works to assist people in northern Rockingham County to reach their full potential, by providing service coordination and family support services. Through our Futures programs we provide trusted direct supports for high school transition, education, employment, residential and community services. Further program choices are available from 17 agencies that subcontract with One Sky.

HOME CARE PROVIDER – Generous Tax Exempt Stipend – Portsmouth Area

One Sky Futures is in search of a home/ Home Care Provider for a 63 year old woman who suffered a traumatic brain injury approximately 8 years ago. She has rebounded a great deal in that time, learning to walk and talk again. At the time of her accident she was a single mother working 2 jobs. A lot of those independent characteristics remain with her.

She has a full 30 hour day program with an established and very trusted staff, Mon-Fri 9am-3pm.

Ideally, we are seeking a Home Care Provider with a home on the Seacoast. She has currently been living in the Raymond-Deerfield-Candia communities; but would love to be closer to Portsmouth area. First floor bedroom or handicap access to her living space would be required.

As a result of her accident, she has a slow gait and can be unsteady on her feet – she ambulates very slowly using a cane in the home and a walker in the community. While she does ambulate independently, she should have someone close by if there are steps, uneven terrain, icy/slippery conditions or while she’s getting used to a new place. She can do stairs, very slowly and needs someone to walk in front of her. She will use her cane and a railing.

While she does toilet independently, she struggle at times with “clean up”. A lot of sensitivity is required, as she is very proud and may be embarrassed if she makes a mess. She requires assistance with ADLs, and oversight for safety, especially until she is comfortable in her new environment. Once a relationship is built, the Home Care Provider’s assistance can be faded to oversight with cues for task completion.

She enjoys animals, especially horses; being social, art, crafts, and many other activities. Being involved in the kitchen is important to her. She used to cook all the time and would love to share a home with a Home Care Provider who can help foster her love of cooking for her again. She has strong family involvement – her 3 children take turns taking her out on most Sundays and holidays. Family is extremely important to her. She would ideally like to feel a part of a household and have a mutual respecting relationship with whom she lives with. *Preferred not to have small children in the home.*

Compensation is in the form of a monthly tax-exempt stipend. Minimum monthly documentation required.

GROUP HOME DSPs NEEDED – ON CALL PER DIEM –  North Hampton & Newmarket

Work with people with physical disabilities and behavioral challenges. Seeking someone experienced with personal care for individuals with quadriplegia, using Hoyer lift, wheelchair lift and tie downs in a handicapped van, communication devices and standers.

The DSP will provide assistance to the individuals in the home and community to develop and enhance basic living skills, personal decision making, social skills, safety skills, a healthy lifestyle including good nutrition, and rights and responsibilities as citizens.

Some of the essential job functions are (additional duties will be explained during interview process):

  • Ensure health & safety of individuals
  • Implement Individual Service Agreements
  • Provide personal care & hygiene
  • Perform housekeeping duties
  • Provide administrative duties

DIRECT SUPPORT PROFESSIONALS – School to Work Day Program – PT – Newmarket, NH

One Sky Futures (a service provider department of One Sky Community Services) is a provider of direct care services to adults with developmental disabilities and/or acquired brain injuries. We have a school to work day program , located in Newmarket, NH. This program is looking for reliable PART TIME (M-F afternoons), enthusiastic, self-motivated and energetic Direct Support Professionals (DSP) to support the people we serve.

Each client is unique and has their own interests, passions and personality which requires Direct Support Professionals (DSP) that are able to respect each clients choices and support them in multiple activities and environments.

Prospective Direct Support Professionals (DSP) candidates need to be flexible and committed to supporting the clients in a way that allows them to reach their maximum potential.

As a Direct Support Professional (DSP) dependability is priority! Experience given consideration but dependability, commitment and a positive attitude more important. Must be open to providing complete personal care in some cases – training is provided.

Direct Support Professionals (DSP) will be provided with training and must pass a NH Criminal Background Check, BEAS Check and Driving Record Check. Responsibilities include:

  • Configure and implement application features within the agency

A Portsmouth, NH family is seeking someone to assist in the care of their 12 year old son with autism. Duties would include assisting in activities of daily life such as: bathing, toileting, tooth brushing, cleaning his room, cooking/baking, practicing social skills in the community, and from time to time going to the pool to practice swimming – with flotation devices.

The benefits of this position are the joy of helping a loving, kind, young boy with autism learn everyday skills. Experience working with individuals with autism is a plus, but we will train the right candidate.

A family in Greenland, NH is looking for someone to provide a variety of daily responsibilities and activities with and for our 22 year old daughter with developmental disabilities.  Position responsibilities include some meal prep, job support, transportation, schedule coordination and engaging in peer social and/or community activities.  Activities vary per day and per week.  Our daughter is kind hearted and pretty consistently upbeat.  She loves singing to the radio, following baseball stats, swimming, cooking, crafts and enjoys staying busy.  We are looking for someone who can interact with her as the young adult she is developmentally, while supporting her navigating through her week, including her work.

Hourly wage is $12-14/hr.  Schedule is M-Thurs in the range of 7:30a-3:30p for a total of 23-26 hrs/week.

We are looking for an enthusiastic, energetic, creative, and compassionate person to join a young woman from Nottingham NH who is affected by a developmental disability. You will be providing one-on-one home and community-based supports. You must be willing to explore opportunities wherever they may lead, enjoy meeting new people, and believe people with disabilities can and should lead full happy lives.

This position requires a high school degree or equivalent. The ability to speak, read, and write English proficiently as documentation is required. The ideal candidate must be flexible and able to work with minimal supervision. It would be beneficial to have experience providing direct support and be able to adapt to changing environments. Transportation is required for this program and will include travel throughout the surrounding areas. This is a 20 hour per week program Monday thru Friday. Scheduling has potential to be flexible but this young woman needs to be driven to work by 4pm on Mondays and Wednesdays. Rate of pay will depend on experience.

A family in Northwood, NH is seeking a caring and positive role model for an 18 year old young man with developmental disabilities.

Duties include picking this young man up from school and taking him to pre-planned after school activities 9-15 hours per week, 3 hours per day.

Experience with behavioral issues preferred.  Client’s interests include going to the gym, fishing and being outdoors.
Ideally, looking for a positive male role model who can provide positive behavior protocols.
Hourly wage is $13/hour plus mileage.

In addition to assisting in personal care and therapeutic exercises, you will assist in participating in community activities.  Ideally, you have experience working with people with disabilities, using a hoyer lift, stander, and assisting in transportation in a wheelchair van safely, but we will provide training for the right candidate(s).

Experience preferred though willing to train the right person for this 20 hour per week position.  Successful candidate will be allowed to choose 20 hours per week from the following days/times: Monday-Thursday 3p-7:30p, Friday 1p-8p and/or Saturday 12p-8p.  Hours worked per week is not to exceed 20.

Meetings are held from 5-7 in the evening, on the second Monday of the month,10 times a year (light supper served) at our Portsmouth office. We should have twelve members on our Board. One Sky Community Services is a New Hampshire private not-for-profit corporation, one of ten Area Agencies designated by the state for the purposes of planning, creating and maintaining a comprehensive service delivery system intended to maximize the community integration, growth and independence of people with developmental disabilities in the 24 cities and towns of upper Rockingham County. We provide some services directly, and also provide funding to agencies such as the Richie McFarland Children’s Center, Easter Seals, Great Bay Services, and Living Innovations.
